Dog licence form

The $30,00 licence fee for each dog must be paid before March 15 of every year. For applications made between March 15 December 31, the fee is $40,00. Once payment has been made, the medal and the receipt will be mailed to the address below.

Please note that a maximum of two (2) dogs are authorized per address.
